Enrollment & Tuition
All students applying to PTSEC must submit:
- Completed application
- Photocopy of birth certificates for each student
- Immunization Record (Form DH 680)
- Health Examination (Form DH 3040)
- Withdrawal form and copy of student's records (if coming from a public school)
- Withdrawal letter and a copy of student's records (if coming from another private school)
- Copy of IEP (if applicable)
- Copy of legal guardianship (if applicable)
- Termination Letter From Home Education Program (HEP) -if your child was enrolled in your county's Home Education Program
